- 9. Pre-Employment Drug Test Result & Clearinghouse Query (§ 382.301): Negative verified drug test result and full Clearinghouse query consent/result.
- 10. CDL License Verification & Self-Certification Proof: Clear, uncropped copy of current commercial driver's license and state medical self-certification confirmation.
